After scheduling an appointment with Linda via email or phone call, please download, fill out, and bring the following packet to the first appointment. The packet includes the following, as well as other important documents:

  • Child/Adolescent Intake (PDF)
  • Notice of Privacy Practices-HIPPA (PDF)
